• Extract from the maternity hospital documents set for the child

If the child and mother are healthy, they are discharged from the hospital on the 4-5th day after birth. The child is discharged only after BCG is given to the child on the 3rd day after birth, audio screening is performed on the 3-4th day, and blood is taken from the child for neonatal screening on the 4th day of life.

If the child is discharged from the maternity hospital on the 3rd day (at the request of the mother or due to overcrowding in the maternity hospital), he will have to undergo audio screening and neonatal screening on an outpatient basis. i.e., a newborn child will need to be taken to the clinic for examination, which is much less convenient than doing everything at once in the maternity hospital.

An extract from the maternity hospital is planned the day before, but the mother always finds out for sure whether she is discharged or not on the day of discharge. 2 doctors report the discharge: the obstetrician-gynecologist reports the mother's discharge, the pediatrician reports the child's discharge. Each of these two doctors may delay discharge by a few days.

Discharge from the maternity hospital is carried out in the afternoon (after 12.00) after the doctors make a round and prepare documents for discharge.

Documents upon discharge from the hospital

On the day of discharge, the mother is given documents

    For the registry office

Child's birth certificate- to register the child. It indicates the date of birth of the child, the name of the mother. With it, you need to contact the registry office where the child will be registered and a birth certificate will be issued to the parents.

    For local pediatrician

    1. Statement of health of the child (Exchange card, information about the newborn). It indicates: Date of birth of the child, full name of the mother, features of the course of pregnancy and childbirth, weight and height of the child at birth and at discharge, head and chest circumference, the assessment of the child on the Apgar scale, when the child was put to the breast and on what kind of feeding, are made vaccination records (two of them are made in the maternity hospital: against viral hepatitis B and against tuberculosis), records of neonatal and audiological screening. An extract on the state of health of the child is the first document from which his outpatient card will begin.
    2. Birth certificate. This document was issued to the mother at the antenatal clinic after 30 weeks of pregnancy and after 12 weeks of regular monitoring at the antenatal clinic. Upon admission to the maternity hospital, she handed it over together with an exchange card, upon discharge it is returned to her, but not all of it, but only 2 coupons: 3-1 and 3-2. With these coupons, the child will undergo mandatory free examinations in the children's clinic from birth to 1 year. This includes ultrasound, ECG, medical examinations and laboratory tests.

The birth certificate and an extract on the child's health will be taken from the mother by the local pediatrician at the first examination of the newborn at home. On the day of discharge from the maternity hospital, the pediatric nurse sends information about the child by phone to the children's clinic, which includes the address where the child will live. If the mother indicated the correct address, then the local pediatrician and honey. the sister will visit the newborn within three days after discharge without an additional invitation.

    For women's consultation

At discharge, the woman is returned her exchange card + an extract on the features of the course of childbirth and the postpartum period. She must provide both to the antenatal clinic, where she was observed before childbirth, within 3 days, after discharge from the hospital.

Set for a child to be discharged from the maternity hospital

While mother and child are preparing for discharge from the hospital, father must prepare things for mother and child for discharge and an apartment for their return home. In practice, the future mother prepares things for herself and the child in advance.

The simplest set for a child to be discharged is described

But now it is fashionable not to wrap the child in diapers and a blanket, but immediately put it on blouses, pants, etc. Then the child will need a suit, a hat and an envelope or overalls.

And, of course, you always need a diaper (preferably 2), corresponding to the weight of the child (size 0 or 1),

Discharge from the hospital in the summer

If the child is discharged from the maternity hospital in the summer when the air temperature outside is 24C and above. The child can be dressed in a diaper, a bodysuit or a light undershirt, a thin knitted jumpsuit and a light hat or cap, and then put it in a light (summer) envelope, or wrap it in a knitted diaper, or put on 1 more light elegant summer overalls.

Extract from the hospital in autumn, winter, spring

If the child is discharged in the spring, autumn or winter, you need to:

    • Put on a diaper, then a bodysuit or a blouse (a bodysuit is more convenient than a vest, because they do not ride up), then a blouse + pants or cotton overalls, a light hat or cap.
    • Next, a warm hat and overalls are put on the child, or wrapped in a blanket, or placed in a warm envelope. How warm a blanket, overalls or envelope should be depends on the weather.
    • In winter, an additional item may be required between the first and second points: a warm blouse and pants or a warm (flannel, flannelette, knitted) diaper.

If you plan to carry a child in a newborn seat, he must be dressed in overalls or in a special envelope with sleeves and a slit between the legs for fastening straps. There are also very comfortable transformer overalls that easily turn from an envelope with sleeves into a jumpsuit. An ordinary envelope or blanket is not suitable for transporting a child in a car seat.

    Solemn discharge from the hospital

How many people to invite for discharge are decided by mom and dad. A solemn discharge is needed for dad, grandparents and other relatives and family friends who want to admire the baby and celebrate a joyful event. The child needs rest. Mom, tired after childbirth and the hospital, wants to quickly get home and is not ready for a long and noisy holiday. This should be taken into account by relatives.

Solemn discharge (the part that takes place within the walls of the maternity hospital) may include: a toastmaster who leads the process and delivers a solemn speech, photographing, video filming.

An extract, even a solemn one, is made quickly, it takes 20-30 minutes. Mother and child are escorted to the changing room.

Mom first changes clothes herself and puts herself in order.

Then the nurse of the children's department dresses the child. If the mother ordered, the process of dressing the child is recorded on a video camera and his first photo session is held.

Next, mom and honey. a sister with a child in her arms goes out into the hall for a solemn discharge. Here their relatives and toastmaster are waiting for them. Tamada delivers his speech, honey. the sister gives the child to the father, the father (if he sees fit) gives her flowers and gifts. You can invite the doctor and midwife who delivered the baby to the discharge and thank them, as well as take a picture with them. Photographs and videos are taken throughout this process.

Further, the mother, the child and a group of happy relatives go out into the street and further actions are limited only by the flight of their imagination. Most often, the group is photographed several times at the walls of the maternity hospital with their camera, then everyone gets into cars and goes home.
